Michael, you might want to clarify what is the business context here and how/when/what for exactly you are planning to use this, as well as what information specifically needs to be filled in. The devil is in the details here. I might be wrong, but it appears that choice of verbiage in the original post might have caused some misunderstanding.
As a side note, there is a lot of information already available on the Adobe forms (I'm guessing you'll need to search for interactive forms) as well as on using Word files in SAP. Use Google.
